INSTRUCTIONS FOR COMPLETING CRIMINAL HISTORY RECORD INFORMATION (CHRI)

1. Complete and sign Criminal History Record Information (CHRI) Volunteer/Prospective Employee form
2. Complete and sign DPS Computerized Criminal History (CCH) Verification form
3. Attach a copy of: Texas Driver’s License or
   1. Texas State ID or
   2. Out of State Driver’s License or
   3. International Driver’s License

NOTE: The photograph on the document presented must be clear and recognizable.
